If you have pain or an emergency situation, every attempt will be made to see you that day.
We try our best to stay on schedule to minimize your waiting. Due to the fact Dr. Nizam and Dr. Ivory provide many types of oral surgery services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.
Office Hours
Deposit & Cancellation Policy (Cosmetic Appointments)
- Cosmetic Consultation Fee with Dr. Nizam: One time, non refundable fee of $100.00 charged at the time of booking. Virtual Consultations are $200.00.
- Cosmetic Consultation Fee with PA/Nurse: One time, non refundable fee of $100.00 charged at the time of booking. Applicable towards services.
- Skin Services Consultation Fee with Estheticians: One time, non refundable fee of $50.00 charged at the time of booking. Applicable towards services.
- We require a credit card on file during the time of booking cosmetic appointments.
- Cancellation/Reschedule or No-Show Fee: 50% of scheduled procedure for estheticians and $200.00 for appointments with Dr. Nizam/PA/Nurse. Appointments must be confirmed by 1pm the day before.
- Late Fee: If running later than 15 minutes & affecting another appointment after, service will be cancelled and the cancellation policy above will apply.
